01 - Place pears, apple if using, apple cider, water, and lemon juice in a large saucepan over medium heat.
02 - Add cinnamon sticks, cloves, nutmeg, and ginger. Stir thoroughly to distribute spices evenly.
03 - Heat the mixture until it reaches a gentle boil, stirring occasionally.
04 - Reduce heat to low, cover the saucepan, and simmer for 20 to 25 minutes until fruit is very tender and flavors are fully infused.
05 - Remove from heat. Pour through a fine mesh strainer or cheesecloth into a heatproof pitcher, pressing firmly on fruit to extract maximum liquid.
06 - Stir in honey or maple syrup while hot, adjusting amount according to personal preference.
07 - Pour into mugs and serve warm, optionally garnished with a cinnamon stick or fresh pear slice.
